Abstract

ABSTRACT Al-Ahmad Husada Clinic is a clinic located in Kayen Pati District. The clinic is one of the places for treatment that is widely used by the community because it is easy to reach. Some people in the village prefer to go to the clinic for treatment because of the relatively close distance, in addition to the "suitability" factor for treatment which according to some people consider a doctor or clinic appropriate when handling patient complaints. The process of inputting patient data to recording medical records here is still done manually by registration officers and doctors by recording all activities in the clinic in a notebook. To facilitate medical records, the authors built an information system to help archive all patient data for treatment and all medical record data to facilitate all those involved in the patient treatment process. Data collection methods are interviews and observation. The tool used to build an information system is PHP as a programming language with a codeigniter framework to speed up system creation and MYSQL as a database server. The information system will assist the service process which includes patient registration, disease diagnosis, treatment, and periodic reports. Keywords: Clinic, Medical record, PHP, MYSQL..
