Abstract

The Yogyakarta City Veterinary Polyclinic is an agency under the auspices of the Yogyakarta City Agriculture and Food Service which is used as a means of providing animal health services. In this Animal Polyclinic, medical record data recording is still done manually, thereby reducing work and service efficiency. A web-based medical record information system is one of the right solutions to overcome problems that occur at the Yogyakarta City Veterinary Polyclinic. With a computerized recording system, the process of managing data and creating reports will become easier, faster and more precise. The system creation process starts from analyzing the running system. The results of the analysis are converted into document flow which is divided based on existing transaction processes. From this document flow, a new system was developed which was then written into the system flow. By implementing this system at the Yogyakarta City Veterinary Polyclinic, it is hoped that it can reduce recording errors that may occur. This system is also expected to speed up the process of making reports which in the end can help decision-making parties to further improve existing performance and services.
