Abstract

IMPLEMENTATION OF A WEB-BASED MEDICAL RECORD APPLICATION (Case Study: Korpagama Clinic, Universitas Gadjah Mada) Abdul Gafur, Murti Renotwo, S.Kom., M.Cs. Ringroad Utara, Jombor, Sleman, Yogyakarta Email: gafura891@gmail.com, murti.retnowo@uty.ac.id ABSTRACT Korpagama Clinic is a medical facility that provides health services and health consultations. The registration process and medical record management at Korpagama Clinic are currently conducted manually, using physical documents or books for record-keeping. The development method utilized in this research is based on the System Development Life Cycle (SDLC) and Data Flow Diagram (DFD). The research involved analyzing and observing the needs of users, namely the public for online registration, doctors with features for examinations and prescriptions, and administrators with features for adding patient data, doctor data, medication data, and polyclinic data. The implementation of the web-based medical record application at Korpagama Clinic aims to facilitate the processing of patient medical records by both the administrators and doctors. This web-based medical record system application is developed using PHP programming language and MySQL as the database. Testing conducted using the Black Box method resulted in a success rate of 100%, indicating that the application is suitable for use. Through analysis and development, the authors conclude that this application is beneficial for the public registration process and for managing and storing patient medical record data at Korpagama Clinic, Universitas Gadjah Mada.
