Abstract
Independent Practice of Midwife Euis Tita Kartika S.Tr Keb is a maternity home in Tasikmalaya City. Currently, patient medical records are still recorded manually at this facility. This manual recording method often leads to challenges, such as difficulties retrieving patient data and medical records when needed. Such issues can impede the delivery of prompt and accurate health services. Therefore, developing a computer-based medical record system is essential to address these problems. This computerized system will be built using PHP (Hypertext Preprocessor) as the programming language and MySQL as the database server. This system is anticipated to facilitate the recording and management of patient medical record data, making the process easier, faster, and more organized. Additionally, the system will minimize the risk of data loss and enhance the efficiency of searching for patient information. Implementing this system will assist midwives and medical personnel provide more effective and efficient health services at the Independent Practice of Midwife Euis Tita Kartika S.Tr Keb.
