Abstract
ABSTRAK Penelitian ini bertujuan untuk menganalisis potensi bahaya kecelakaan kerja pada proses preventive maintenance boiler di PT. PLN Indonesia Power UBP Suralaya dengan menggunakan metode Hazard Identification, Risk Assessment, and Risk Control (HIRARC) serta Failure Mode and Effect Analysis (FMEA). Proses pemeliharaan boiler memiliki tingkat risiko tinggi karena melibatkan aktivitas di ruang terbatas, suhu dan tekanan tinggi, serta mesin-mesin bergerak. Berdasarkan metode HIRARC, ditemukan 27 potensi bahaya dengan kategori risiko yang bervariasi, di mana beberapa di antaranya termasuk dalam kategori risiko ekstrem. Selanjutnya, metode FMEA digunakan untuk memberikan penilaian kuantitatif terhadap skenario kegagalan melalui parameter Severity, Occurrence, dan Detection, sehingga diperoleh nilai Risk Priority Number (RPN) untuk setiap potensi bahaya. Hasil analisis menunjukkan bahwa potensi bahaya dari mesin yang bergerak memiliki RPN tertinggi sebesar 250, yang menjadi prioritas utama untuk dilakukan pengendalian. Risiko-risiko lain dengan RPN tinggi antara lain terjepit kipas blower, kebisingan ekstrem, dan bahaya listrik. Kombinasi metode HIRARC dan FMEA memberikan pendekatan yang lebih menyeluruh dalam mengidentifikasi, menilai, dan mengendalikan risiko kecelakaan kerja. Hasil akhir dari penelitian ini menghasilkan rekomendasi pengendalian risiko berdasarkan prioritas, yang diharapkan dapat menjadi dasar dalam penyusunan strategi keselamatan kerja yang efektif dan berkelanjutan di lingkungan pembangkit listrik. Kata Kunci: K3, Boiler, HIRARC, FMEA, Preventive Maintenance ABSTRACT This study aims to analyze the potential hazards of workplace accidents in the boiler preventive maintenance process at PT. PLN Indonesia Power UBP Suralaya using the Hazard Identification, Risk Assessment, and Risk Control (HIRARC) and Failure Mode and Effect Analysis (FMEA) methods. The boiler maintenance process has a high level of risk because it involves activities in confined spaces, high temperatures and pressures, and moving machines. Based on the HIRARC method, 27 potential hazards were found with varying risk categories, some of which were included in the extreme risk category. Furthermore, the FMEA method is used to provide a quantitative assessment of failure scenarios through the parameters of Severity, Occurrence, and Detection, thus obtaining a Risk Priority Number (RPN) value for each potential hazard. The analysis results indicate that the potential hazards from moving machinery have the highest RPN of 250, making them a top priority for control. Other risks with high RPNs include trapped blower fans, extreme noise, and electrical hazards. The combination of the HIRARC and FMEA methods provides a more comprehensive approach to identifying, assessing, and controlling workplace accident risks. The final results of this study yielded prioritized risk control recommendations, which are expected to form the basis for developing an effective and sustainable occupational safety strategy in the power plant environment.
