Abstract
The development of information technology that is very fast now has an important role in all activities, so that supporting performance for various agencies becomes effective and efficient. Health aspect is one of the important aspects for life. Al-Barra Mother and Child Hospital which is a private health service center in Mukomuko City is one of the developing health institutions. The number of patients who are increasing every day causes quite severe problems in getting information about patients, hospital drug data, and data on patient visits that are available or currently being used. Al-Barra Mother and Child Hospital still uses the method of service and medical records manually or in the form of bookkeeping. Data collection and archiving all drug data and patient data is an important thing to note. Based on these things to create an effective and efficient service and patient medical record, good management is needed by making a service information system and patient medical records so that it can find out the number of patients who have registered or patients who visit medical treatment, as well as home drug stock data the hospital, the Al-Barra City Mukomuko Maternal and Child Hospital will be built a web-based information system using the waterfall method, with stages namely needs analysis, program code design and writing, program implementation and program testing. The software that will be used in building this information system is HTML, Bootstrap and PHP as programming languages, Apache as a web server, and MySql as a database server. The service information system and patient medical record, is a system that will produce information and provide convenience to patients when registering, treating, and providing convenience for the Clinic in processing patient data and drug data, so that it will produce complete data for the Mother Hospital and Al-Barra's child.
