Abstract
Information technology becomes a necessity for the society including in the health aspect. In public health centers or Puskesmas, information technology can be used to develop an information service system. The problem is that most of the management of medical records in Indonesia are still using spreadsheet applications. It makes them record manually in paper form. Spreadsheet application also has a weakness when the officers of the public health center should understand the formulation to use it but actually they are not programmers. It happens in the Public Health Centre in Pulau Temiang, where medical records are written on paper and they booked it for archives. This is less efficient because the data can be lost or damaged. This study aims to facilitate the process of managing medical record data such as storing and accessing data and to reduce the risk of data damage due to disaster. This application is designed as web-based application by using PHP programming language (Pear Hypertext Preprocessor) and MySQL as its database. Rapid Application Development (RAD) is used as the methodology for system development in this study. The result of this application is a web-based health center service information system that can facilitate the data searching, modifying, and reporting. This application is expected to facilitate the implementation of activities in public health centers and improve services to the society. Key words:, Health Services, Health Centre, Web.
