Abstract
ABSTRACT Information is necessary for a clinic. Fast, precise, and accurate information bring the best results for the clinic itself, either in the performance of the clinic with high effectiveness and efficiency or in providing the best service for patients. Sido Waras Primary Clinic has many kinds of services, such as general medical service, dental, and obstetrics examinations. One of the main problems occurs at the Sido Waras Primary Clinic recently is the recording data of the patients’ medical records. It is still using manual records that require more space and costs for storing patient medical record data. Searching for medical record data is not easy and taking long time because it needs to search manually. Therefore, this research was conducted to solve those problems. A new system development was made to make it easier in managing data and information such as computerized data storage processes and services. The purpose of this research is to develop an information system for the Sido Waras Primary Clinic in controlling data and providing faster services and information. The method used to solve those problems was the Waterfall method in which the completion and system development was carried out in stages. The test results of the system created were the system running according to the user who used it and each user was related to each other to produce correct information. This information system was built using the PHP programming language (Pear Hypertext Preprocessor), MySQL as a database, and Sublime Text 3 as a programming support tool.
