Abstract
Tulakan Public Health Center (Puskesmas) is a government agency that serves the community in the field of health located in Bungur Village, Tulakan District, Pacitan Regency, East Java. As one of the centers of public health services, the recording of patient data is a special need that must be done. One of the problems is that connectivity between service units is poor because patients register by filling out a form sheet then each unit must input it’s own data on the same patient, starting from the beginning of registration, examination, diagnosis, action, up to the report. So in the event of an increase in the percentage of patient visits often results in the build-up of queues at the registration counter and examination of patients. The system is expected to be able to solve the problem of patient data collection in Tulakan Health Center, ranging from initial data recording of patient registration, data search to the creation of patient medical records report quickly and precisely. Because the system is website-based, the PHP programming language (Pear Hypertext Preprocessor) with MySQL and CSS data storage media (Casading Style Sheet) is chosen to get a more user friendly system view. Waterfall method is used as an approach to the development of systems that run gradually and sequentially so as to minimize errors in system development.
