Abstract
Puskesmas Moyudan is one of the public health center on Jl Tempel-Klangon, Ngentak, Sumberagung, Moyudan, Sleman, Yogyakarta. Puskesmas Moyudan has a patient data management system that does not use internet technology and is not computerized. All data is still recorded in the physical form. In connection with these conditions, the authors built a website that concentrates on creating or designing simple applications that are tailored to the needs of Puskesmas Moyudan. The purpose of this study is to design and build a web-based system or application to handle patient data management at Puskesmas Moyudan which can provide accurate, optimal and easy information in making patient data reports. In this study, the authors used the observation method to collect each data, the authors conducted direct interviews with resource persons at Puskesmas Moyudan to obtain accurate data. The process includes patient registration, examination and treatment, the resulting system is a diagnosis, hospital and doctor's prescription. This information system can help process patient data at Puskesmas Moyudan.
