Abstract
ABSTRACT The background for the establishment of this system is because one of the puskesmas located in the village of Kalipancur when providing health services for villagers is still done manually, such as when residents want to register for an examination, residents must come directly to the puskesmas to get a queue number. The storage of puskesmas data such as patient data, health care data and patient medical record data is still carried out in a book so that it makes admins find it difficult when they need these data and it takes a long time to find the required data. Sometimes data loss and writing errors are common. The purpose of this system is to overcome the problems that occur in the puskesmas and can help the admin in performing services at the puskesmas such as in processing puskesmas data so that it can be faster, stored neatly and safely. And can make it easier for patients to submit examinations and medical records. This system will be designed using the PHP programming language and the system will be made online so that patients can access the system anywhere. The results obtained from this system include the system being able to submit examination queues, request medical records, send messages from patients to admins, store user data, health care data, patient data and medical record data. As for reports, the system is able to print reports on patient data, health workers and medical records. Keywords: Puskesmas, Health, Web, PHP
