Abstract

USULAN PERBAIKAN SUMBER BAHAYA MENGGUNAKAN METODE HAZARD IDENTIFICATION RISK ASESSMENT AND DETERMINING CONTROL DAN FAULT TREE ANALYSIS Studi kasus: UD Sinar Harapan Muhammad Riski Syafriansah 1, Ferida Yuamita 2 Program Studi Teknik Industri, Fakultas Sains dan Teknologi, Universitas Teknologi Yogyakarta ABSTRAK UD. Sinar Harapan merupakan UMKM yang bergerak dibidang industri manufaktur kayu. Produk yang dihasilkan semuanya memakai bahan baku dari kayu. UD. Sinar Harapan berdiri pada tahun 2000. Pada UD. Sinar Harapan telah terjadi kecelakaan kerja sebanyak 3 kasus kecelakaan kerja. Pada tahun 2018 telah terjadi kecelakaan kerja sebanyak 1 kasus dan pada tahun 2019 telah terjadi kecelakaan kerja sebanyak 2 kasus dengan 3 jenis kecelakaan kerja seperti, tangan tertusuk kayu (1 kali), pada bagian mata terkena serpihan kayu kecil (1 kali) dan tangan sterkena palu (1 Kali). Tujuan dari penelitian ini adalah untuk mengindetifikasi faktor-faktor yang menyebabkan kecelakaan kerja dan menentukan langkah yang tepat dapat meminimalisir tingkat kejadian atau kemungkinan kecelakaan kerja di UD. Sinar Harapan. Metode Hazard Identification, Risk Asessment, and Determining Control (HIRADC) bertujuan untuk mengetahui bahaya apa saja yang ada pada perusahaan tersebut. Selanjutnya hasil dari penilaian risiko yang masuk dalam kategori high risk akan diprioritaskan untuk diiidentifikasi lanjutan menggunakan metode Fault Tree Analysis (FTA). Setelah dilakukan identifikasi dan pengolahan data menggunakan Hazard Identification Risk Assessment And Determining Control (HIRADC) maka diketahui bahwa ada 1 potensi bahaya dengan tingkat high dan 3 potensi bahaya dengan timgkat medium dan selanjutnya diidentifikasi menggunakan metode Fault Tree Analysis (FTA). Sinar Harapan is an MSME engaged in the wood manufacturing industry. The products produced all use raw materials from wood. UD. Sinar Harapan was established in 2000. At UD. Sinar Harapan, there have been 3 work accidents. In 2018 there have been 1 case of work accidents and in 2019 there have been 2 work accidents with 3 types of work accidents such as a wood stabbed in the hand (1 time), small wood chips in the eye (1 time) and a hammer hit in the hand. (1 time). The purpose of this study is to identify the factors that cause work accidents and determine the right steps so as to minimize the incidence or possibility of work accidents at UD. Ray of hope. The Hazard Identification, Risk Assessment, and Determining Control (HIRADC) method aims to find out what hazards exist in the company. Furthermore, the results of the risk assessment that fall into the high risk category will be prioritized for further identification using the Fault Tree Analysis (FTA) method. After identifying and processing data using Hazard Identification Risk Assessment And Determining Control (HIRADC), it is known that there is 1 high level hazard potential and 3 medium hazard potentials and then identified using the Fault Tree Analysis (FTA) method.
