Abstract

ABSTRACT Advances in information technology have significantly impacted the healthcare sector, particularly in the management of chronic diseases such as diabetes. The Pratama Bandar Lor Clinic in Kediri continues to face challenges in managing patient data, especially in recording medical histories, monitoring blood sugar levels, and facilitating communication between patients and doctors, which are still conducted manually. To address these issues, the DiaCare application was developed, a web- and mobile-based diabetes patient management system designed to enhance healthcare efficiency. This application includes features such as medical history recording, blood sugar and HbA1c monitoring, medication reminders, online consultations, diet and nutrition guidance, and digital delivery of test results. This research employed a prototyping method using Flutter technology for mobile application development and Firebase for backend services and data storage. Black-box testing demonstrated that all application functions operated flawlessly, achieving 100% success without errors. Consequently, the application enables patients to self-monitor their health and improves the effectiveness of clinic services. This system is expected to streamline and integrate the diabetes patient management process, supporting real-time communication between patients and healthcare professionals.
